Use the Consent to Recording Sessions (V1 08.2025) form when a family requests or agrees to photos or video recording during sessions.

This form allows recording for clinical review, supervision, and staff training and can be withdrawn by the parent or guardian at any time.

When to Use

• Family requests photos or videos

• Provider wants to record sessions for training or review

Important

• Do not record without a signed form

• This consent is not included in standard intake paperwork

• Upload the signed form to Mission Control (MC) following current documentation workflows
